AFFIDAVIT of SUPPORT & RESPONSIBILITY

I, hereby declare under penalty of perjury that the following statements are true and correct to the best of my knowledge and belief:

1. 1. 1. 1. I am the person responsible for my invitee, who is a national of the Republic of Cuba (“Cuban National”).
         2. The Cuban National will be traveling from Cuba to the Dominican Republic and will return to their country of origin within the timeframe permitted by law and/or the period granted by their visa.
         3. The purpose of the trip is exclusively for tourism in the Dominican Republic and to allow me to spend time with the Cuban National.
         4. The duration of the trip shall not exceed thirty (30) days.
         5. I expressly guarantee that my invitee will not remain in the Dominican Republic beyond the period authorized by the Ministry of External Affairs of the Dominican Republic. Furthermore, the invitee will not seek to establish residency or change their immigration status while in Dominican territory and will engage solely in the activities for which the visa has been granted.
         6. I ACCEPT FULL MORAL AND FINANCIAL RESPONSIBILITY FOR THE RETURN OF MY INVITEE  TO THEIR COUNTRY OF ORIGIN. I FURTHER UNDERTAKE TO COVER ALL EXPENSES INCURRED DURING THEIR STAY IN THE DOMINICAN REPUBLIC, AS WELL AS ANY COSTS ARISING FROM THEIR REPATRIATION OR DEPORTATION.
         7. This responsibility is assumed not only in favor of the Dominican Republic, but also in response to any legal claim that may be brought in the United States by Dominican authorities, their legal representatives, or any attorney acting on behalf of the airline or travel agency involved in the sale of this family reunification travel package.
         8. I ALSO AGREE TO ASSUME FULL RESPONSIBILITY FOR ANY LEGAL COSTS OR RELATED EXPENSES IN THE UNITED STATES RESULTING FROM SUCH CLAIMS.
         9. I fully understand that a visa is a privilege, not a right. As such, it must be earned and respected. I acknowledge that the issuance of a visa is subject to the sole discretion of the competent authorities, and there is a risk that the visa may be denied regardless of this affidavit, or any other supporting documentation provided. I also understand and accept that, in the event the visa is not approved for any reason, the fees paid for the visa application are non-refundable under all circumstances.
         10. I acknowledge and accept that neither YES Travel, nor any associated travel agency, airline, or hotel involved in the travel package bears any responsibility whatsoever for the visa application process. The visa is a personal process that must be handled individually by the applicant. YES Travel only acts as a recipient and transmitter of supporting documentation but does not process, evaluate, approve, or assume responsibility for the outcome of the visa application under any circumstance.
